In-Home Physical Therapy

The Homecare physical therapy service is individualized and helps patients to build strength and regain balance in order to enhance safe functional mobility within home surroundings. Our trained therapists help patients increase their ability to perform the activities of daily living (ADLs), improve communication and swallowing techniques in conjunction with family education to maximize their independence within the safe home environment.

The Silvercare Homecare team collaborates with therapy and nursing services and establishes a personalized plan of care for each patient to fulfill each individualized need.

In-Home Physical Therapy is a service in which a state-licensed physical therapist comes to visit you to provide services instead of you going to a clinic. This can help you get the treatment you need for your pain and injuries when it’s too risky for you to leave your home.
You will need to have a face-to-face visit with a referring physician such as a family doctor or be discharged from a hospital or nursing rehab center within 30 days. A case manager will start with the homecare service per your medical need and arrange the nursing and rehabilitative services that address your areas of concern.
In-Home Physical Therapy varies and is dependent upon your medical need and your progress. The typical duration will last about 4-6 weeks. In-Home Physical Therapy sessions last about 30-45 minutes per visit.
Within your home environment, the physical therapist will provide gait training, stairs negotiation, endurance programs, therapeutic exercise, therapeutic activities and balance training as well as family support education and / or training per your need.
